Patient Rights & Responsibilities

You have the right to: 

  • Considerate and respectful service
  • Obtain service without regard to race, creed, national origin, gender, sexual preference, age, disability, diagnosis or religious affiliation
  • Make informed decisions about your care and the medications or supplies we provide you.
  • Confidential handling of your personal health information. Individuals or organizations not involved in your care may not have access to the information without your consent.
  • Reasonable continuity of care and service.
  • Voice grievances without fear of termination of service or other reprisal in the service process.

You have the responsibility to: 

  • Promptly report any problems with the medication and/or supplies.
  • Notify us if you have not received your order within 14 days of shipment.
  • Use the medication and/or supplies in compliance with the physician’s order and in a safe and proper manner, following proper storage and dosing instructions.
  • Notify us of any change in health, insurance, address, telephone number, physician or prescribed use.
  • Notify us upon admission to a hospital, skilled nursing facility or home care agency.
  • Accept financial responsibility for medication and/or supplies furnished by Natura Pharmacy for any charges not covered by your insurance.
